Honestly, these "my T is low, do I need TRT?" posts are tough to answer when there's almost no background info.
Tell us a little more about yourself. Are you active? Do you lift? How's your diet? What's your lifestyle like? What have you actually done so far to try and address the low T besides making a post here?
At 6'1" and 270 lbs, I'd guess you're carrying a pretty high body fat percentage, possibly 30%+. If that's the case, hopping on TRT may not be as simple as you think. Running 120 - 160 mg/week while carrying that much body fat can sometimes create its own issues, especially if you're a high aromatizer. You may end up having to keep an eye on estrogen levels as well.
The one positive thing is your SHBG is looking low, which can be helpful. But what other labs have you had done? How's your thyroid? What does your LH, FSH, prolactin, IGF1, DHEA, Cortisol, 25-OH (D) HCT, HGB and HbA1C at?
There's just not enough information here for anyone to tell you whether TRT is the answer. Low testosterone can be caused by a lot of things, and in some cases improving body composition, diet, sleep, and overall health can move the needle quite a bit.
Need a lot more info than "Hey guys, my T is low, should I start TRT and run an AI?"
Also, I'm just not down on what seems to be the increasingly common approach these days of blindly jumping straight to a "Test and Reta" protocol as well.
