TRT Beginner.

Hello, I’m thinking of starting TRT. I will post my blood work. I’m currently 33, 6,1” tall and 270 lbs. I have lower test and higher estrogen levels. What would be the best way to start and how much? Do I need to take an estrogen blocker right away since it’s currently high? Should I start around 150-200 a week of test.

My trt clinic started me at 160 a week split in two times a week, .25 arimidex 3 times a week, then they just after bloods at 6 weeks
 
Honestly, these "my T is low, do I need TRT?" posts are tough to answer when there's almost no background info.

Tell us a little more about yourself. Are you active? Do you lift? How's your diet? What's your lifestyle like? What have you actually done so far to try and address the low T besides making a post here?

At 6'1" and 270 lbs, I'd guess you're carrying a pretty high body fat percentage, possibly 30%+. If that's the case, hopping on TRT may not be as simple as you think. Running 120 - 160 mg/week while carrying that much body fat can sometimes create its own issues, especially if you're a high aromatizer. You may end up having to keep an eye on estrogen levels as well.

The one positive thing is your SHBG is looking low, which can be helpful. But what other labs have you had done? How's your thyroid? What does your LH, FSH, prolactin, IGF1, DHEA, Cortisol, 25-OH (D) HCT, HGB and HbA1C at?

There's just not enough information here for anyone to tell you whether TRT is the answer. Low testosterone can be caused by a lot of things, and in some cases improving body composition, diet, sleep, and overall health can move the needle quite a bit.

Need a lot more info than "Hey guys, my T is low, should I start TRT and run an AI?"

Also, I'm just not down on what seems to be the increasingly common approach these days of blindly jumping straight to a "Test and Reta" protocol as well.

But here's the flipside to what I said, TRT can be the kickstart some people need. If low T is dragging down their energy and motivation, getting optimized can make it much easier to get active, lose weight, and make the lifestyle changes they've been struggling to stick with.

What drives me crazy is when people jump on TRT and don't change a single thing else. If you're not putting in the work to improve your health and lifestyle, you're leaving a lot of the extremely potential benefits on the table. We're not about that shit here.

But yeah... 296 is a low number for sure, and ask yourself is pinning yourself for the rest of your life is the answer, and if you're ready to commit to that.

I'm noticing alot of men with low T these days. Way lower than it was 10-15yrs ago. I've got 2 18yr old boys who work for me that just got labs done and their T levels were in the 300s at 18. They eat right, train hard and work hard and really it's not affecting them it seems. But all the low T candidates these days is unbelievable.

Yeah, alot of it is environmentally based... the microplastics and other endocrine disruptors.
 
My first test was 195.

This was after I had lost maybe 30lbs and had been working g out regularly. What made me question my test level was I didn’t have that drive to workout like I once had in my 20’s. Working out just took so much from me and dreaded doing it at that point. I was not getting used to it as I had in the past.

Once I got my test levels checked and got things optimized it was night and day. My energy went up and my body composition continued to improve. Now it’s at a point where if I don’t workout I feel off. Even if I’m sick, I still feel off just laying around.

In hindsight, I have to eat crow on this one. If you're genuinely doing everything right and your testosterone levels still aren't where they should be, I'd strongly recommend TRT. At some point, it's not about taking shortcuts, it's about treating a real deficiency.
 
If necessary TRT done right is life changing.. this is the place to be. I avoided a nightmare and was informed at all times thank guys especially Cypher.. lol

A note to consider I’m pretty sure being in a cut/long deficit will run your natural test levels down.. as least for me it did.. Not training but training hard in a deficit ..

Also yeah my Tsh/fsh were just pinned whether I was 250 or 350 TT So yeah TRT balls we’re cooked.

I always recommend starting low, re-test in 12 weeks. If at 100mg per week your test levels reach 700 range you are good to go, if not add another 50. Not need to start high. Just my opinion. Best of luck to you.
 
I was 31 when I had my dr check it and I was about the same except for the high estrogen. Dr refused me to a urologist and he has kept my test at 1000 for me. They will start you low and work your way up if needed, every one is different so you might just need a little or you might need more just take it slow and take your time.
