Testosterone >1600....Anyone on Gear experience exhausting Fatigue?

Background story first.
Age 62 and from the compliments everyone young hopes they look like me at 62... but I'm small.... 5'2" 158-162lbs,
Been on gear for about 5 years now with normal cycling, as far as I know.... 4 months on 4 months off.

Had my blood work in May 2025... PSA levels more than doubled from last year 2024....

2024 PSA - 3.4
2025 PSA - 7.8
Next step was seeing a urologist.
More blood work for some type of cancer screening... came back with high probability of prostate cancer.
Wife was freaking out wants a full PET SCAN.
Unknown to me the Doctor should have done an MRI, apparently that's text book flow chart but he went right to the BIOPSY.
12 clicks of the BIOPSY and found cancer but with additional testing came back with a Gleason 6 score which means very unaggressive and unlikely to spread... it can but it's not likely or common.

Now with all of these blood work they never checked my Testosterone levels and I've had really bad fatigue for the past 3-4 months going on with no idea why.

The doctors don't know I'm on TEST, TREN or other gear no need for it in my chart- don't ask/ don't tell.

So I keep seeing these ads for men with low energy can get your testosterone checked... there mills to sell you Viagra- luckily for me I have no problem in that dept and don't need it.

I went to get my testosterone checked thinking maybe they could also tell me why the fatigue.

PSA levels 5.6 less than the spring- no idea why but I see the urologist in 2 weeks so I'll ask then and prob do more blood work at that time to check again.
Test came back >1600 - normal ref range for over 50 yrs old 216-1004. They said they have no idea why the Fatigue.

Does anyone else here on Gear experience exhausting Fatigue?

Probably time for me to go off cycle and see how that affects my Fatigue.
 
E2 is probably high and at a level that your body doesn't like or agree with, this will cause the fatigue you are experiencing. If I were you, I'd just cruise until blood work and assess the situation and do what your bloods tell you to do.

I’d drop down to true TRT, 10mg test cyp daily subQ is the best way to get higher free testosterone and lower estrogen conversion, according to a big thread by an old IFBB PRO on professionalmuscle.
 
I have Test E....I'll order test C...hope it gets here this week.

Test C 10mg...how much ml daily and for how long approximately?
 
I am 65 as the years go by i feel better with test levels that are not to high. Growing and maintaining muscle later in life is harder for the body to do as it is not as young as it used to be when everything was easy and i could recover almost over night.
 
DHT feeds the prostate and can cause the swelling dudes worry about. If you are preventing the aromatization of excess test to estrogens but not to DHT, then that can be a problem. There are some that think by blocking conversion to estrogens, the test converts more to DHT.
Something to consider.
Another might be to stop the "cycling: and just stick with TRT. Plenty can be accomplished with just TRT.

A lot going on here and not much good coming out of it. Probably a good idea to stop cycling, for now, for sure, and make sure you get your health in order brother. The "hope I look like you at 62" compliments wouldn't be worth it if I ended up in the hospital soon and didn't make it to 65.
 
Test C 10mg...how much ml daily and for how long approximately?
I’ve never tried it myself. I prefer intramuscular injects. But, blood tests from the guys trying the subQ daily showed higher free testosterone and less estrogen conversion. They say the long acting ester injected daily subQ gives best results. Cyp and enanthate are basically the same (one molecule difference) so you can do it with enanthate if you want.
I, personally, feel like subQ shots cause swelling which looks like crap! I suppose you could inject it under the skin anywhere.
If you have 200mg testosterone then you would take 5ius daily on a slin pin to get 10mg testosterone. If you have 250mg testosterone then take 4ius to get 10mg testosterone. I’d use a 27 gauge 1/2” pin, but you can go smaller gauge if your testosterone uses MCT oil as the carrier oil.
They do this year round for TRT.

Very likely unrelated to your test levels. How recovery, sleep, stress, diet etc? Where’s e2?

And he thinks the swap from test e to test cyp is going to solve anything. I hope he pumps the breaks hard. Sounds like he's pretty lost

Not trying to be a dick @Zumbadaddy but it's pretty clear you need to get off the gear, get your health in order, and do a lot more reading before doing anything besides minimal trt doses

I'll go with just the Test and stop the TREN and NPP. And schedule blood work for E2.

Diet is very clean.
Recovery? I work same body parts every 3-4 days with good gains I'm satisfied with. Not sure that answers the question.
Sleep has been bad for years prob due to ADHD..I'm typing this at 2am since I woke up to pee and I'm up.
Stress is not bad compared to others....I'm not really stressed a lot.

How much of each compound are you on weekly (mgs)?

When getting e2 make sure it’s ultra-sensitive since you’re on tren. It will skew a standard estradiol test.

You need to fix sleep brother, as I’m sure you know.

@JJB1, would a person need to wait 4-6 weeks before testing to see if there was a change ? Or longer ? This is something I thought about trying because I’m a pretty high converter, Thanks!
 
@Zumbadaddy The advice already given I can't ad to. Good advice. The prostate thing I have a questions. You said it's Gleason 6 so I'm assuming 3+3? And did they do genomic testing? My genomic testing came back at less than 2% chance of advancing. The medical community has been arguing 50/50 about declaring Gleason 6 (3+3) to not even be cancer but benign. I'm not saying forget about it by any means. I'm pretty sure they have you on "active observation" to keep an eye on it regardless. What I'm saying is try not to lose sleep over it. They told me I'll die with it not from it and advancement or metasticizing is next to impossible. And they skipped the MRI with me as well. I didn't know any better out of ignorance and when I found out it pissed me off. The MRI is to check for anomalies BEFORE doing a biopsy so IF they find anything it can be a targeted biopsy instead of the random 12-15 punctures. My dad had to have it done and I made sure he was well aware of proper procedure so he didn't have to go through that. Anyway. I'll be on active observation until I reach an age where it's irrelevant. The whole point of this being don't get too worked up over the prostate thing. Your psa will fluctuate. Just keep an eye on it. It will more than likely never change from where it is now.

IMO, running too much will not actually make your body better over the long haul. It’s a trade off…short term muscle mass (a lot from water retention) for other trade offs. So just don’t run that much gear.

Are there any experts in the field that recommend finasteride or doutesteride to reduce DHT? While on test based steroids?

There are negatives to these 5-alpha reductase inhibitors, just like any drug.
Some people use nutritional supplements first to see if they work. Saw palmetto is one iirc.
 
Finasteride can cause ED issues.
Even if you’re cruising at 200 + HCG?

Fin lowers DHT by 60% and on cycle running 500 + wouldn’t you have large amounts (way over reference range) and taking Finn would technically put you on top or slightly over the reference range. So you would have the same dht of a normal healthy male who’s not on peds or a little more.
