Most of the population thinks that gynecomastia can only be caused by steroids, either due to a high presence of estrogens or an elevation in prolactin due to the use of Nandrolone or Trenbolone (completely false myth), gynecomastia can also be due to a malfunction of the liver.

The liver metabolizes and conjugates more than 50% of estrogens, and it is done mainly by two phases (Phase 1 and Phase 2 pathways).
When the liver is damaged the metabolism of estrogens is altered, both within the liver and in the extra hepatic tissues, causing an elevation 
of the level of estrogens and therefore causing gynecomastia
