Hard to tell since it's an old pic and hairy lol but I'd guess that was about 165lbs lean mass around 10-13% bf. That said, I think your diet is really close. May even want a little more carbs but I'd start exactly where you're at. I personally would swap the potatoes for oats in the morning. Think that's the only change I would make.
The only factor we don't know is how much muscle you've lost. It may be negligible it none if you didn't abuse any substances or alcohol.
You could even bump your cruise dose up a little or add some deca if your joints bother you from pushing hard. Other than that the only thing I'd add is a little GH at night until you get a little leaner and decide you want to blast
